Boarding and Daycare Agreement
business name takes pet name (pet breed, pet age) from guest name for service from drop off date to pick up date. Signed date.
1. Health. pet name is current on the vaccinations business name requires and is free of fleas, ticks and contagious illness. The owner will tell staff of any condition, medication or behaviour that staff should know about: pet notes.
2. Behaviour. The owner confirms that pet name has not bitten a person or seriously injured another animal, other than as noted here. An animal that threatens people or animals may be separated, and the owner called to collect it.
3. Care. business name provides food, water, shelter, exercise and supervision as described at the counter. Food from home is given as instructed here: feeding instructions.
4. Veterinary care. A separate Veterinary Authorization says what business name may do if pet name is ill or injured and what the owner will pay.
5. Collection. Animals not collected within abandonment days days of the pick-up date, after business name has tried to reach the owner and the emergency contact, are treated as abandoned under the law where business name operates.
6. Payment. rate per day, payable at collection, plus veterinary costs incurred under the authorization.
By signing the owner confirms that they own pet name or are authorised by the owner, and that they have read this and meant to sign it.
