A salon or spa consent covers a beauty service. Anything that crosses into medical aesthetics — injectables, medical-grade peels, laser under a physician — is a different category with different rules about who may consent, who must supervise and what the record must contain, and we are not going to describe this as a fit for that just because it can hold text and a signature.

Questions

Can the consent record whether a patch test was done?

Yes, as a field on the document with the date, and the client signs what they chose. That is a much better record than a note in a calendar, because it carries the client's own signature against the answer.

Do clients have to sign again every visit?

For a chemical service, most salons want a fresh consent each time because the history and the plan change. For a standing treatment you can set a period the acknowledgment covers instead.

What about photographs for social media?

A separate permission with its own answer, which a client can decline without declining the service. The answer is a field your team can read rather than something somebody has to remember at the mirror. Taking a later change of mind is still yours to handle: the record here is of what was signed, and there is no withdrawal door in the product today.
