Veterinary Authorization
Owner: guest name. Animal: pet name. In the care of business name from drop off date. Signed date.
1. If pet name is ill or injured. business name will try to reach me on owner phone and then my emergency contact, emergency contact on emergency phone. If neither can be reached and staff judge that pet name needs care, I authorise business name to take pet name to vet name or, if unavailable, to the nearest veterinarian.
2. What I authorise. Examination, and treatment up to spending limit without further contact. Treatment above that amount needs my agreement, unless the veterinarian says delay would cause suffering or death, in which case I authorise what the veterinarian judges necessary.
3. Cost. I will pay the veterinary costs incurred under this authorization directly to the veterinarian or reimburse business name at collection.
4. My own vet. vet name, vet phone. pet name's known conditions and medications: pet notes.
By signing I confirm that I own pet name or am authorised by the owner, and that I meant to sign this.
