Guardian Consent for a Minor Shooter
I, guardian name, am the parent or legal guardian of guest name, born date of birth, and I sign this on date for their use of the range at business name.
1. Consent. I consent to the minor shooting at the range, and I have read the range waiver and safety rules. I agree to their terms on the minor's behalf to the extent the law where business name operates allows a guardian to do so.
2. Supervision. I understand that the minor must be supervised on the line by a responsible adult at all times, and that the adult doing so today is responsible adult, who is present and has signed a waiver of their own. Supervision means within arm's reach.
3. Instruction. The minor has been instructed in the four safety rules and in the range's own rules, or will be before they shoot, and I accept that a range safety officer may stop them at any time.
4. Eyes and ears. The minor will wear eye and hearing protection at all times past the line marked.
5. Health. The minor has no condition that would make shooting unsafe, other than any I have written here: health notes.
6. Medical care. If the minor is injured and I cannot be reached, I consent to first aid and to emergency medical care, and I am responsible for its cost.
7. Contact. I can be reached on guardian phone while the minor is at the range.
By signing I confirm that I am the minor's parent or legal guardian, that I have read this, and that I meant to sign it.
